Prescription Renewal Policy

To ensure safe, evidence-informed prescribing in accordance with the College of Nurses of Ontario (CNO) standards, prescription renewals require clinical review. If a prescription renewal is requested without a scheduled assessment and can be safely authorized based on a review of your medical record, current medications, recent investigations, and clinical history, an administrative fee of $20.00 per prescription will apply. This fee reflects the professional time required to assess the appropriateness and safety of continuing the medication and to prepare and transmit the prescription to your pharmacy. Prescription renewals cannot be guaranteed without an assessment. If your medication requires monitoring, laboratory testing, dose adjustment, follow-up for a chronic condition, or if there have been changes to your health status, you will be required to schedule an appointment before a renewal can be authorized. Requests should be submitted at least 5 business days before your medication is due to run out to allow adequate time for review. Renewal fees are not insured services and are the responsibility of the patient.

Cancellation

If you need to cancel your appointment, please do so 48 hours before your scheduled appointment so that someone else can have your appointment time. Cancellations made within 48 hours may be subject to a $50 fee.

Late Policy

I understand that life happens. Please be respectful of this time, and appointment time may be shortened or necessitate re-scheduling.

No Shows

If you do not show for your appointment, a fee of $50 will be applied to your credit card on file, and another appointment will not be booked until this is paid.

Visit Length

Appointments will end at the scheduled time out of respect for the next patient that is scheduled. Please ensure you book the appropriate appointment with the associated time that fits your concern or another appointment may be necessary.

Fees

I cannot provide direct billing however an invoice will be provided which can be submitted to your insurance provider. Fees will be deducted from your credit card unless another form of payment is preferred.

Controlled Substances

Occasionally, controlled substances may be medically necessary for treatment. However please note that these will not be prescribed for no longer than 30 days at a time. You will also be requested to sign a controlled substance agreement, which is a practice standard.
